ChARM PHR: A Patient Portal

ChARM PHR is a Personal Health Record (PHR) portal that allows you to access and manage your health information.

After your first appointment with me, I will send you an email invitation to register for the PHR portal. 

Once registered, you can use the button below to log in to your portal account to:

• access all of your visits and any documents that are shared by me;

• send secure messages to me; and

• manage your health records.
